The rates below are for OSU customers. Services for qualifying other agencies (Local, State, Federal) are provided at 125% of regular rates. Commercial services for qualifying users are provided at 150% of regular rates.

The same forms offered in Milne 206 are available for download below in PDF format. These forms can be filled in on a computer or printed and filled in by hand.

FAQ and User Guide

Below is a list of common questions regarding test scoring services and the work order form that are asked on a regular basis. For a more detailed explanation on test scoring services, as well as instructions for filling out the work order form, please read the Test Scoring User Guide (PDF).

 

What do I write on the exam keys?

What do I write on the exam keys?

In the last name section of the exam key, write EXAM KEY and fill in the corresponding bubbles. Example (JPG). For multiple exam keys, complete an exam key for each test form. Blacken in the Test Form Number in the space provided for each answer sheet. Students must also blacken in the test form number to receive credit.

 

What does the A,B,C,D mean next to the report options?

The report letter codes on the test scoring work order form refer to how you want your reports to be sorted. Circle one letter code per report, or don't circle any letter code if you don't want that report. For examples of the kind of reports available for scored tests, visit the test results page.

A

List all sections together - do not separate by form number.

B

List all sections separately - do not separate by form number.

C

List all forms/keys separately - do not separate by section.

D List all forms/keys separately - separate by section within form number.

 

Can the test forms be mailed back to my department? 

Yes. Write your campus mailing address in the Return Instructions box. The original test forms along with any printed reports will be sent to your department.

 

Can I receive separate grades for different sections of the test?

Yes. Option 10 on the second page of the work order form is where a test can be divided into sub sections. A grade will be given for each sub section along with a grade for the entire test.

 

How do I indicate that some questions are worth a different amount of points?

Option 11 on the second page of the work order form is where individual questions or a range of questions can be set to different point values. The point values indicated in this section will override the point value assigned by option 8 on the first page. Any questions not indicated in variable point section will be assigned to the point value given in option 8.

 

How do I indicate that multiple answers are correct for a question?

Option 12 on the second page of the work order form is where a question can be assigned multiple correct answers with different point values if desired. Write the form/key number and question(s), and then write the point value for each correct answer. You do not need to indicate a 0 for the incorrect answers. Only blacken one answer per question on the exam key.

 

Can I throw out a question so it isn't graded?

Yes. Use option 13 on the second page of the work order form to assign point values to questions that you do not want scored. For example, throwing out a question and assigning it a point value of 2 will give everyone 2 points for that question, regardless of what answer they've chosen.

 

How do I make a question extra credit?

Use option 14 on the second page of the work order form to designate questions as extra credit. Students that answer questions marked as extra credit will receive a point value if their answer is correct, but an incorrect answer will not affect their overall score.

 

Where do I get blank scan sheets?

If your department does not provide general purpose answer sheets to instructors, answer sheets can be purchased in Milne 206 or purchased online. Both methods require a FIS billing number.

Test Results

The results of a scored test can be sent electronically via email or printed at no additional cost. Test reports are emailed in a Microsoft® Word or PDF format. A raw data file (.DAT) and a Microsoft® Excel file can also be emailed if requested on the Test Scoring Service Work Order form.
